Step One: Do you have Acrobat Reader on your computer?
Want to use forms from this website? You’ll need the Free Acrobat Reader that can be downloaded from their website by clicking on the blue hyperlink above. Next, scroll to the bottom of their front page and click on the yellow box labeled "Get Acrobat Reader." Follow the easy-to-do directions. Quality will be the same as an original. Step Two: I’ve got Acrobat Reader, what’s next?

• Click on the form you want to use. The form will open automatically, and you will see it on your computer.

Option One - print the form as it appears

• Choose the ‘Print’ option from the ‘File’ menu or click on the ‘Print’ icon.

Option Two - download the form to your computer

• Right click on the form.
• Select "Save Target/Link As.." when presented with a menu.
• Save the form where you want it to live on your computer.
