We have gathered these forms from several sources. We have also edited, written and combined several forms together ourselves.
Examining the various forms that lawyers used was instructive in that there was no "standard operating practice." Some forms were clearly designed for the lawyer or legal assistant to complete. Others were for the client to complete. Some lawyers used one form for all information. Others used one form for contact information and another form that differed depending on the specifics of the case.
No one practice appears to be superior. It is most important that the practice be followed and the forms completed. We have also included a separate conflicts checking form which may be used as a stand alone form or incorporated into a larger form.
